  • Understanding discharge summary: reading and interpreting key medical terms
  • Vital signs monitoring at home: temperature, respiratory rate, heart rate (age-appropriate ranges)
  • Weight monitoring: using infant scale, tracking growth charts, when to be concerned
  • Oxygen saturation monitoring (if prescribed): using pulse oximeter, normal ranges
  • Recognising deterioration: colour changes, respiratory distress signs, feeding changes, lethargy
  • Medication administration: oral, inhaled (nebuliser/inhaler with spacer), eye drops
  • Follow-up appointment management: keeping track of multiple specialist visits
  • Home safety: safe sleep, safe feeding, preventing falls
